ElderSafe Case Managers are qualified, experienced and professional Nurses, registered with the College of Nurses of Ontario and BC.
Through referrals, or an individual’s request, a Case Manager will attend your home to do a health and risk assessment. Then, in consultation together with the client and family, will determine the types of services required to enable the client to maintain as much independence as possible. We will then create and implement a care plan specifically designed for your needs… and wants. This care plan will be revisited regularly to ensure you are getting the best help possible.
It is also important to note that all personnel have been thoroughly screened with reference checks and Vulnerable Sector Police checks and are required to wear photo-identification at all times.
